Homelessness Outreach
We go directly to the streets, encampments, and shelters of Fresno to meet people in their most vulnerable moments. Our outreach team brings not just supplies, but presence — a reminder that no one is invisible, no one is forgotten, and every person is chosen.
What We Provide:

Nourishing bodies, restoring dignity.
Food & Clothing
Hunger and lack of clothing are immediate, urgent needs. Through our mobile pop-up events and direct outreach, we distribute hot meals, non-perishable food items, and clothing to individuals and families across Fresno — no questions asked, no judgment given.
What We Provide:
- Hot meals at pop-up events
- Non-perishable food bags
- Clothing for men, women, and children
- Hygiene kits and personal care items
- Seasonal items (blankets, coats, etc.)
Caring for the whole person.
Health & Wellness
Access to healthcare is a right, not a privilege. We connect community members to health screenings, wellness resources, and local medical services — bringing health information and referrals directly to those who need it most through our mobile outreach model.

- Connection to free/low-cost clinics
- Basic first aid and wellness supplies
- Chronic illness awareness and education
- Coordination with local health partners

Mental Health & Suicide Prevention
Mental health crises don't wait for office hours. Our team is trained to walk alongside individuals experiencing depression, anxiety, trauma, and suicidal ideation — offering compassionate presence, crisis resources, and the unwavering message that their life has value and purpose.

Public Safety Support
We believe safe communities are built on trust. Chosen Ones Rehab works to bridge the gap between residents and public safety by fostering relationships, reducing tension, and creating pathways for cooperation — because everyone deserves to feel safe in their own neighborhood.

Faith & Prayer
Faith is the foundation of everything we do. We offer prayer, scripture encouragement, and spiritual support to anyone who wants it — never forcing, always offering. We believe that spiritual healing is inseparable from physical and emotional wholeness.

Mobile Pop-Up Events
Until we establish a permanent facility, we take everything on the road. Our mobile pop-up events are held the 2nd Saturday of every month at the corner of Parkway & Dennett in Fresno. Regular hours are 12:00 PM; summer hours (June–August) are 10:00 AM.

Encouragement & Mentorship
Sometimes the most powerful thing we can offer is a voice that says: you are seen, you are valued, you are chosen. Our mentorship and encouragement ministry walks alongside individuals on their journey — celebrating progress, offering accountability, and never giving up.
